FHIR Research – The Jerusalem Center

The Jerusalem Center for Computational personalized medicine in The Hebrew University, The Hadassah University Medical Center, Shaare Zedek Medical Center, Meuhedet HMO, Outburn Ltd.

Distributed data research, joint by three organizations, in the field of obstetrics and high-risk pregnancy.

As part of the research, the data shall be standardized throughout the three organizations, enabling the creation of continuous clinical data derived from multiple and varied sources: prenatal care, diagnosis, and postpartum hospitalizations, from early pregnancy through delivery.

The group’s study case

  • Distributed data research (federated learning), joint by 3 organizations, on obstetrics and high-risk pregnancy. As part of the research, a standardization is made throughout the three organizations, enabling the creation of continuous clinical varied and structured data (prenatal care, diagnosis, postpartum hospitalization), over the time span, from early pregnancy through delivery.

  • The current research is the first milestone in the broader project, which is aimed to provide a universal and wider platform for collaborations on further research.

The project’s world of content

  • High risk pregnancy

  • Pregnancy and birth

  • Cross-reference of data for research

  • Data-intensive research

  • Distributed databases

  • longitudinal digital medical record

  • Mother clinical states which effects the newborn

Solution Architecture

As part of the project, a FHIR server will be established in each and every organization in order to populate data, as the research requires.

Populating the FHIR server in data, will be done using the integration tools of the organization (e.g. ESB or ETL or other designated tools).

Data requirement for any research, activates the anonymization automatic unit (according to the research approved application), and then transfers the data to the control of the federation system which performs the algorithm calculations and shares results with the other organizations in the research.

The results share process activates the federation algorithm again, this time to make changes into the statistical estimations and to accurate the results as a response to the new information received from the partner organizations in the research.

Profiles to be characterized in the project

  • Patient (Mother, Child)

  • RelatedPerson (Mother)

  • Observation (Pregnancy Summary, Pregnancy History, Delivery, Pregnancy Findings, Blood Pressure, Umbilical Cord, Newborn Findings, Newborn Body Weight, Newborn Head Circumference, Lab Result Mother, Lab Result Newborn),

  • Encounter (Delivery, Pregnancy, Birth),

  • Condition (Past Mother Diagnostics, Pregnancy Diagnoses, Delivery Diagnoses, Umbilical Cord Complications, Newborn Diagnoses),

  • Procedure (Delivery, Delivery Procedure, Complication Treatment, Birth, Newborn Complication),
